Description

Be part of a thriving outpatient program, with a collegial environment and an emphasis on clinical training and development.  SunBridge Harbor View Community Services Center in Long Beach has an immediate opening for a full time Case Manager 1 in our Full Service Partnership (FSP) program. The Center is an outpatient mental health program for children ages 5-21 in Long Beach, CA.  We are contracted with the Los Angeles County Department of Mental Health to offer services to children with Med-Col in the Long Beach area.  We currently serve over 350 children with our combined services, including field-based mental health, Therapeutic Behavioral Services (TBS), and the new Full Service Partnership (FSP) programs.

The Case Manager 1 position is available in our field-based, FSP program.  The Case Manager is responsible for treatment of the Center’s clients enrolled in its field based mental health programs by providing case management and behavioral interventions for the Center's clients in multiple settings such as their homes, at school, in the community, as well as at the center.  The Case Manager is also responsible for serving in the role as a Parent Partner by providing direct support services, education and advocacy for families within our program by using their own experiences with a child with mental health problems to assist in addressing and tailoring interventions to the specific needs of the family.
