Registered Nurse Case Manager

One of the region's fastest-growing health plans is looking for an experienced Registered Nurse Case Manager who is seeking an exceptional career opportunity with the benefit of working from the comfort of his/her own home on a regular basis.

Training is required at the NYC Corporate offices.  After training, this position telecommutes from home most of the time, with visits to the office as needed for additional meetings and training.

In this position you will:

• Contribute to the development of patient-focused care and initiate the process in accordance with established guidelines
• Conduct assessments, develop treatment plans, perform defined interventions, and generate referrals to providers, community-based resources, and appropriate social services
• Assess member's physical, psychosocial, and discharge planning needs through communication with appropriate hospital staff including attending practitioner, utilization review staff, discharge planners, social workers, and coordinate care accordingly
• Function as a clinical resource / educator for the multi-disciplinary health care team on an ongoing basis in order to maximize quality of patient care as well as effectively manage length utilization and coordination of resources
• Assist in identifying opportunities for and facilitating alternative care options based on member needs and assessments
• Evaluate and pre-authorize request for inpatient/outpatient, specialty care, home care, DME, and transportation services from network and out-of-network providers in accordance with departmental guidelines
• Document all interventions and telephone encounters with providers, members, and vendors in the appropriate system in accordance with established documentation standards to ensure integrity of member services provided over the continuum of care and over time

Please be advised that we are only considering nurses with the following:

• Current RN Licensure in New York State
• Bachelor's Degree in Nursing
• Minimum 3 years clinical case management experience with behavioral health involvement and a demonstrated ability to make contributions to patient-focused care
• Intermediate Excel and advanced Word/Outlook skills
• Knowledge of the concepts and philosophy of case management, utilization review regulations, and relevant medical standards of practice
• Ability to apply nursing processes and systems theory to problem-solving activities
• Good writing and excellent telephone assessment skills
• Must reside within 50 mile radius of job location to be eligible for telecommuting position
